STEP I. GATHER APPLICATION
DOCUMENTS
1. DS-11 Application
Click here to create DS-11 Application. After answering all questions, click on the CREATE FORM button at
the bottom of the page. Print two copies of the completed DS-11 Application on one side of the paper. Do not
sign the DS-11 until you are instructed to do so by an acceptance agent (see STEP II).
2. DS-64 Form (Statement Regarding a Lost or Stolen Passport)
Click here to download and complete DS-64 form, statement regarding a lost of stolen passport.
3. Proof of U.S. Citizenship
1). Original or certified U.S. birth certificate OR
2). Your expired U.S. passport OR
3). Original naturalization certificate
4. Passport Photo
One standard 2 x 2 passport photo (color photo with a plain white background) taken within 6 months.
5. Proof of Departure
Two copies of your travel itinerary or flight confirmation for a foreign destination issued by a travel agent or an
airline. If you need your passport within 6 business days or less, your departure date must be within 3
weeks, or 4 weeks when a visa is needed.
A typed company letter on company letterhead stationery signed by a company official is acceptable as proof
of departure for business trips. The letter should include the applicant's name, date of departure, destination,
mode of transportation and the purpose of the trip.
6. Letter of Authorization
Click here to download and complete Letter of Authorization, which gives MD Visa Plus the authority to
submit your application and pick up your new passport. Prepare two copies of Letter of Authorization with your
original signature.
7. Fees, Processing Time, and Payment
1). U.S. government fee is $170. Make your check or money order payable to U.S. DEPARTMENT OF STATE.
Write your full name and date of birth on the check or money order.
2). Execution fee is $25. This fee is paid to an acceptance agent you are going to use (see STEP II).
3). MD Visa Plus service fee. Make your check or money order payable to MD Visa Plus. To pay by credit card,
complete Credit Card Payment Authorization Form and send it with your application.
• 1-2 business days processing - $249 • 3-4 business days processing - $169
• 5-7 business days processing - $129 • 8-10 business days processing - $99
4). Return shipping fee. Add $35 to your payment made to MD Visa Plus for FedEx Overnight return if you
prefer not to include a prepaid envelope for return shipping. Service fee and return shipping fee can be paid
together by one check or money order.
8. MD Visa Plus Online Order Form
Click here to complete and submit MD Visa Plus Online Order Form.
STEP II. GET APPLICATION DOCUMENTS AUTHORIZED
You must appear in person before an acceptance agent (Acceptance agents are at designated Post Offices
and court houses in your local area) with the documents as listed in STEP I and a proof of your identification,
i.e., driver's license. Tell the acceptance agent that you are using an expediting service to "hand carry" your
passport application to a passport agency for an expedite processing.
The acceptance agent will witness you to sign your DS-11 application and seal, in an envelope, the following
documents:
1). One copy of the completed and signed DS-11 application;
2). Completed and signed DS-64 form;
3). Proof of citizenship;
4). One passport photo;
5). One copy of proof of departure;
6). One copy of Letter of Authorization;
7). The $170 check or money order payable to U.S. Dept. of State.
The following items should not be placed in the envelope sealed by the acceptance agent (they need to be
sent to MD Visa Plus outside the sealed envelope):
1). The second copy of the completed and signed DS-11 application
2). The second copy of proof of departure - travel itinerary
3). The second copy of Letter of Authorization
4). Your payments to MD Visa Plus
5). Printed MD Visa Plus Online Order Form
You need pay the acceptance agent $25 execution fee. The sealed envelope will be returned to you.
STEP III. SHIP COMPLETE APPLICATION DOCUMENTS TO MD VISA PLUS
U.S. Passport Agency must receive the dated application within 5 business days. Therefore, we suggest you
to send the following documents to MD Visa Plus on the same day when you have all the documents sealed
by an acceptance agent using USPS Express or FedEx Overnight:
1). The sealed envelope from the Acceptance Agent
2). The second copy of DS-11 application
3). The second copy of travel itinerary
4). The second copy of Letter of Authorization
5). Payment to MD Visa Plus.
6). Printed MD Visa Plus Online Order Form
