Payment Methods

You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.

Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.

Shipping

Send the above documents, including 1) your passport copy and one application form, 2) all required
documents to be authenticated, 3) a copy of printed online order form, 4) your payment (Credit Card
Payment Form must be enclosed if you pay by credit card), and 5) a prepaid airbill or envelope for return
shipping (optional), to the following address:

For SAME DAY RUSH service, your package with complete required
documents enclosed must be received by us before 9:30 AM, CST.
We recommend you to consider using FedEx First Overnight
when you send your documents for SAME DAY RUSH service.

with your application (no regular mail, please).

Copy of Passport and Application Form

Submit a photocopy of the pages of your passport with your photo and signature on it. Send one

The Documents to be Authenticated

Send the following documents to us: 1) the original documents and one photocopy; 2) notarized
documents, one original and one photocopy; 3) state certificate, one original and one photocopy.

Consular Fees & Service Fees Per Document

Before your important documents are used in China, they need to be authenticated by the Chinese
Consulate for your jurisdiction.

The Chinese Consulate in Houston authenticates such documents as birth certificates, death
certificates, power of attorney, marriage certificates, diplomas, adoption application papers, business
licenses and etc., which originated in the following states: Alabama, Arkansas, Florida, Georgia,
Louisiana, Mississippi, Oklahoma, and Texas.
