Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card. Credit card payment is subject to a 3% processing charge.
Shipping
Send the above documents, including 1) your old and new passports, 2) application form with two
photos, 3) a copy of driver's license, 4) a copy of printed online order form, 5) your payment (Credit Card
Payment Form must be enclosed if you pay by credit card), and 6) a prepaid airbill or envelope for return
shipping (optional), to the following address:

Send us your old passport from which an Indian visa is requested to be transferred along with your new
passport.
