Payment Methods

You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.

Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card. Credit card payment is subject to a 3% processing charge.

Shipping

Send the above documents, including 1) your passport, 2) application form with two photos, 3) a copy of
driver's license, 4) employment appointment letter, contract letter, resume and proof of company
registration in India, 5) a copy of printed online order form, and 6) your payment (Credit Card Payment

Collect and Send the Following Documents

A prepaid self-addressed envelope with a tracking number must be provided
with your application (no regular mail, please).

FedEx Express (Additional fees will be charged if a delivery on Saturday or to
Alaska or Hawaii is requested)

We process India visa applications for residents of the following states: Arkansas,
Colorado, Kansas, Louisiana, Nebraska, New Mexico, Oklahoma, and Texas.

Valid Passport

The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 6 months of validity left and have at least 2 blank visa pages.

Application Form and Photos

Send one visa application form, completed and signed, with two standard passport photos. The
application form must be completed and printed online using the link given next.

Proof of Residency

Send a residence proof such as copy of your driver's license, other state issued ID or a utility bill, e.g., gas, electric, or water bill.

Employment Appointment Letter, Contract Letter and Resume

Proof of Registration in India

Submit a proof that the hiring company is registered in India.
