Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money order
or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) passport, 2) application form and photo, 3) a copy of Visa
Notification Form and a copy of Work Permit for Aliens or Foreign Expert's License, 4) proof of jurisdiction,
5) a copy of printed online order form, 6) payment, and 7) a prepaid airbill or envelope for return shipping
(optional), to the following address:

Valid
Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 12 months of validity left and have at least 2 blank visa pages.
In addition to your original passport, a copy of the picture page of your passport is also required.
Click here for additional required documents if you are a non-US citizen, a China born US citizen applying
for a China visa for the first time or with a new passport, or a child who was born in U.S. with at least one
parent born in China and is applying for a China visa for the first time or with a new passport.
Application Form and Photo
Send one visa application form, completed and signed, with one recent passport photo.

Visa Notification Form and Work Permit
Submit a copy of Visa Notification Form and a copy of Work Permit for Aliens issued by Chinese Ministry of
Labor or a copy of Foreign Expert's License issued by Chinese Foreign Expert Bureau along with the
originals (The originals will be returned with visa).
Proof of Jurisdiction
An acceptable proof of jurisdiction includes copy of driver's license, apartment lease
contract, utility bill, phone bill, credit card, bank statement, or a student ID that shows your name and
address.

Consular Fees & Service Fees Per Visa
