You may pay all fees by a single check (personal check, business check, or cashier's check), money order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to pay your fees by credit card. Credit card payment is subject to a 3% processing charge.

Send the above documents, including 1) passport, 2) photo attached signed application, 3) copy of driver's license, 4) printed order form, and 5) payment, to the following address:

Return Shipping Fee Per Package
We process India visa applications for residents of the following states: Arkansas, Colorado, Kansas, Louisiana, Nebraska, New Mexico, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and signed passport. It must have at least 12 months of validity left and have at least 2 blank visa pages.

Application Form, Photo, and Proof of Residential Address
Click here to complete and submit the visa application form online. The visa application form must be printed single sided on two pages and signed inside the box under the photo on the first page and also on the second page. Follow the guidelines below to make sure your visa application form to be accepted:
1. Indian Mission (Consulate): You must select "USA - Houston".
2. Name: The name on your application must be exactly the same as it appears in your passport.
3. Date of Birth: It must be entered in the format of DD/MM/YYYY.
4. Present Address: It must exactly match the address on the proof of address that needs to be submitted.
5. Period of Visa: Enter "10 years".
6. Visa Type: Select "Tourist Visa". Purpose: Select "Individual Tourist".
7. Before submitting, review all your answers carefully as you can't make any change once it's submitted.
Attach one passport photo and send a copy of your driver's license or a utility bill as the proof of address.
Additional Requirements For Minors Under 18 Only
1. The visa application form for a minor must be signed and notarized by both parents on the second page. On the first page, the minor should sign, or use a thumbprint in black ink if the minor cannot write.
