Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card. Credit card payment is subject to a 3% processing charge.
Shipping
Send the above documents, including 1) passport, 2) photo attached signed application, 3) proof of
address, 4) a business letter and an invitation, 5) printed order form, and 6) payment to the following address:

Return Shipping Fees Per Package
We process India visa applications
for residents of the following states: Arkansas,
Colorado, Kansas, Louisiana, Nebraska, New Mexico, Oklahoma, and Texas.
Valid Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 12 months of validity left and have at least 2 blank visa pages.

applying for a visa for the first time or with a new passport.
Application Form, Photo, and Proof of Residential Address
Click here to complete and submit the visa application form online. The visa application form must be
printed single sided on two pages and signed inside the box under the photo on the first page and also
on the second page. Follow the guidelines below to make sure your visa application form to be accepted:
1. Indian Mission (Consulate): You must select "USA - Houston".
2. Name: The name on your application must be exactly the same as it appears in your passport.
3. Date of Birth: It must be entered in the format of DD/MM/YYYY.
4. Present Address: It must exactly match the address on the proof of address that needs to be submitted.
5. Period of Visa: Enter "1 year", "5 years" or "10 years".
6. Purpose of Visit: Use "business" to relate to the business visa.
7. Before submitting, review all your answers carefully as you can't make any change once it's submitted.
Attach one passport photo and send a copy of your driver's license or a utility bill as the proof of address.
Business Letter and Invitation
A letter from your company indicating the business purpose of your visit, probable duration of stay, validity
of visa, places and organizations to be visited and a financial guarantee, and an invitation letter from a
company in India, should be submitted with your application. The validity of visa to be requested (1 year, 5
years or 10 years) should be clearly stated in the letter. Both business letter and invitation letter must be
written on company letterhead.
