Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money
order or credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to
pay your fees by credit card.
Shipping
Send the above documents, including 1) your passport, original and a photocopy, 2) a copy of your
permanent resident card or U.S. visa, 3) one application form with three photos, 4) a copy of printed
online order form, 5) your payment (Credit Card Payment Form must be enclosed if you pay by credit
card), and 6) a prepaid airbill or envelope for return shipping (optional), to the following address:

Send us the original and a photocopy of your valid and signed passport. A copy of your permanent
resident card or valid U.S. visa is also required.
If the applicant is a child of Chinese descent born in U.S., a copy of the birth certificate and a copy of both
parents' Chinese passports and U.S. visas are also required to be submitted.
Application Form and Photo
Send one visa application form, completed and signed, with three standard passport photos. The photos
must be 2x2 with a white or light background and show a full, front view of your face. Click here to
