Methods
You may pay all fees by a single check (personal check, business check, or cashier's check), money order or
credit card.
Make your check or money order payable to MD Visa Plus. Fill out and send Credit Card Payment Form to pay
your fees by credit card.
Shipping
Send the above documents, including 1) passport, 2) application form and photo, 3) invitation letter, 4) proof
of jurisdiction, 5) a copy of printed order form, 6) payment, and 7) a prepaid airbill or envelope for return
shipping (optional), to the following address:

Shipping Fees Per Package

Passport
The visa will be stamped on a visa page in your passport. Therefore, you must send us your valid and
signed passport. It must have at least 12 months of validity left and have at least 2 blank visa pages.
In addition to your original passport, a copy of the picture page of your passport is also required.

for a China visa for the first time or with a new passport, or a child who was born in U.S. with at least one
parent born in China and is applying for a China visa for the first time or with a new passport.
Application Form and Photo
Send one visa application form, completed and signed, with one recent passport photo.

Invitation Letter
The invitation should be written on a company letter head and signed and stamped. You may submit a
scanned copy instead of an original invitation letter.
Proof of Jurisdiction
An acceptable proof of jurisdiction includes copy of driver's license, apartment lease contract, utility bill,
phone bill, credit card, bank statement, or a student ID that shows your name and address.

Consular Fees & Service Fees Per Visa
